You are the person other people lean on.
Caregiving work selects for a particular kind of person and then trains that person further. You notice what others need before they say it. You stay calm when the room is not. You set your own reactions aside for the length of a shift, a surgery, a session, a family meeting, and you do it so reliably that nobody, including you, stops to ask what it costs.
Over time the setting-aside can become permanent. The feelings that were postponed for later never get a later. What is left is often a kind of competent tiredness: still performing well, still reliable, and increasingly unable to rest, enjoy things, or feel much of anything outside of work.
Many caregivers also carry their own histories into the work. Being the steady one is frequently a role that started long before any training program, which is part of why it is so hard to put down. The complex trauma page speaks to where that pattern sometimes begins.
Understanding the theory does not mean you are outside it.
If you work in healthcare, and especially in mental health, you may already know the vocabulary for what is happening to you. Burnout, secondary trauma, compassion fatigue, dysregulation. You may be able to explain the mechanism more precisely than most people could. And it may not have helped at all.
That is not a failure of intelligence. Understanding a pattern and being able to change it are different capacities. The nervous system does not reorganize because it has been given an accurate description of itself. It reorganizes through experience, relationship and repetition, which is why this is still psychotherapy and not a reading list.
There is also a particular loneliness in being the expert. It can feel hard to be a patient when you are used to being the clinician, or to be uncertain in front of someone who does what you do. I try to make room for that rather than pretending it is not in the room.

Nothing is shared with anyone, including an employer, without your written permission, except where the law requires it. We can discuss the limits of confidentiality at the start.

What we actually do.
We begin with how your days are going now, not with a formulation. Sleep, the end of a shift, what you do with the hour after you get home, where the irritability or flatness lands. From there the work often has two strands. One is building capacity, so that you have somewhere to go other than coping harder. That can include somatic work with what your body is already doing.
The other strand is looking at the role itself: where being the steady one came from, what it protects, and what it would take to let it be one mode among several rather than the only one. That can touch anxiety, depression, or older material, depending on what is there.

- It is very common, and it can be some of the most useful work a clinician does. The awkwardness usually fades once we are working with what is actually happening rather than with the theory of it.
